What are your office hours and how can I make an appointment?

Do you accept my insurance?

What should I bring to my child’s first appointment?
For your child’s first appointment, please bring their insurance card, a photo ID, any previous medical records, and a list of current medications. If applicable, bring your child’s vaccination records as well.
What should I do if my child is sick outside of office hours?

What is your vaccination policy?
